** Please note the deadline to apply for the Adaptive Communication Technology CMP Funds has been extended to June 30, 2021**

On May 12, 2020, Cecile Castello, LDH Deputy Assistant Secretary, sent the attached memo and application to all certified nursing homes.  The memo advises nursing homes of the availability of CMP grant funds for purchases of adaptive communications technology that can be used for virtual visits with family and friends and telehealth for residents. Nursing homes are eligible for up to $3000 cost reimbursement for the devices and accessories. 

LDH reached out to LEADER to assist the Department by serving as the fiscal intermediary to allow for the payment of these funds. LEADER will be reviewing supporting documents provided by the approved nursing home or corporation, such as invoices/receipts, so the nursing home can be reimbursed for the amount approved by LDH.
