LEADER is excited to announce that nominations are now open for the 2026 Hall of Fame Awards, our highest recognition honoring long-term care employees and advocates who exemplify excellence in person-directed care.
These awards celebrate individuals who go above and beyond in delivering compassionate care, demonstrating leadership, and making a meaningful impact in the lives of residents, families, and fellow staff. The Hall of Fame Awards are a tribute to those who embody the heart and soul of long-term care.
We invite you to nominate a deserving team member who has made a lasting difference through their dedication to person-directed care, integrity, and exceptional service.

Nominees must meet the following requirements to be eligible for any of the 2026 Hall of Fame Awards:
Must currently be employed in the position for which they are being nominated.
Must be employed by the nominating community/company for at least 6 months at the time of nomination.
Must be able to attend as a registered participant of the March 2026 LEADER Conference.
Criteria considered for awards:
Demonstration of excellence in their daily work
Example(s) of going above and beyond their job duties to serve a resident or team member
Description of what sets the nominee’s performance apart
One word that captures their approach to excellence in performing in their specific field
Consistent demonstration of person-directed care
Feedback shared by residents, families, or team members
NOMINATIONS DEADLINE: December 31, 2025
Nominee must be a Registered Nurse or Licensed Practical Nurse.
Nominee must be an Activity Director.
Nominee must be a Certified Nursing Assistant.
Nominee must be in a Housekeeping, Dietary, or Maintenance role.
Nominee must be in a Social Services or other Administrative role.
Nominee must be an Ombudsman.
